Immediate Care is conducting a fundraiser to give $10 back to the sports or camp program of your choice for each $30 physical performed at one of their numerous locations. Any self-paying patient is eligible for the physicals program for the remainder of 2017.
“Providing sports and camp physicals is just one of the many conveniences we provide busy families with here at Immediate Care,” CEO Sal Cannizzaro said. “By giving back $10 from the cost of each physical to that specific organization, we are also helping to build and foster relationships within our communities – it’s a win for the families and a win for the clubs and teams on which their children participate. It’s just one of the many ways Immediate Care keeps their focus on helping its customers.”
To take part in the campaign, download the donation form at immcare.com/wp-content/uploads/2017/07/ImmedCare-SportsCamp-Flyer.pdf and bring it with you for your physical assessment. Be sure to include the name of the program recipient on the flyer. Also, bring any necessary physical exam forms given to you by your organization for our providers to complete as proof of eligibility for physical activities.
