Somerset County conducting needs-assessment survey for community-based services and programs

The Somerset County Office on Aging & Disability Services is conducting a needs-assessment survey for public input to help plan programs and services for Somerset County residents ages 60 years and above, people living with disabilities, and caregivers. The objective is to gather information about how to improve community-based services and programs. The survey is available in print and online. The deadline for responses is March 31. 

“Everyone’s input is valuable and makes a difference in funding and program decisions made by the county’s Office on Aging and Disability Services,” said Somerset County Commissioner Paul M. Drake, liaison to the Department of Human Services.

“Examples of currently funded programs include paratransit services, home care services, caregiver support, nutrition programs, and the Home-Delivered Meals program. All of these are extremely important to our communities.”

Needs Assessment Survey Information

All information given by survey participants will remain confidential. If there is more than one eligible member in the household, each person should complete their survey. Caregivers can complete the survey for individuals who are unable to complete the form.
